Nature of the evidence base and approaches to guide nutrition interventions for individuals: a position paper from the Academy of Nutrition Sciences

This Position Paper from the Academy of Nutrition Sciences is the third in a series which describe the nature of the scientific evidence and frameworks that underpin nutrition recommendations for health. This paper focuses on evidence which guides the application of dietary recommendations for individuals. In some situations, modified nutrient intake becomes essential to prevent deficiency, optimise development and health, or manage symptoms and disease progression. Disease and its treatment can also affect taste, appetite and ability to access and prepare foods, with associated financial impacts. Therefore, the practice of nutrition and dietetics must integrate and apply the sciences of food, nutrition, biology, physiology, behaviour, management, communication and society to achieve and maintain human health. Thus, there is huge complexity in delivering evidence-based nutrition interventions to individuals. This paper examines available frameworks for appraising the quality and certainty of nutrition research evidence, the development nutrition practice guidelines to support evidence implementation in practice and the influence of other sources of nutrition information and misinformation. The paper also considers major challenges in applying research evidence to an individual and suggests consensus recommendations to begin to address these challenges in the future. Our recommendations target three groups; those who deliver nutrition interventions to individuals, those funding, commissioning or undertaking research aimed at delivering evidence-based nutrition practice, and those disseminating nutritional information to individuals.

Objective
The aim of this paper was to provide a systematic review and update on the available longitudinal studies on the impact of prenatal alcohol exposure (PAE) on language, speech and communication development, as well as associated potential environmental confounders during the preschool period.
MethodsA literature search was restricted to English, full‐text, peer‐reviewed, longitudinal studies in from 1970 until present: PUBMed, Scopus, Web of Science {C-e Collection, Biological Abstracts, KCI-Kean Journal Database, Russian Science Citation Index, SciELO Citation Index, Zoological Rec-d}, Academic Search Premier (Africa-Wide Information, CINAHL, MEDLINE, PsycINFO. Keywords included: prenatal alcohol exposure (PAE); speech or language or communication outcomes; neurocognitive or neurodevelopment or neurobehavioral or neurobehavioural; infant or baby or toddler or preschooler; longitudinal or follow-up. The inclusion criteria included (i) longitudinal cohorts with at least 2 time-points; (ii) association of light, moderate or heavy PAE on language, speech or communication delay, development or disorder; (iii) environmental confounders; (iv) infants up to preschool age.
ResultsSix studies satisfied the threshold for inclusion. Three studies reported that PAE was significantly associated with receptive or expressive delay. These studies demonstrated lower scores on either receptive or expressive communication in the alcohol group in comparison to the non-alcohol group, even after controlling for environmental factors up to 36 months.
ConclusionEvidence from the longitudinal studies reviewed suggest that PAE influenced delays in receptive and expressive communication up to 36 months. Contextual risk factors played a significant role in language development over time and especially as children approached school age.
Being occupied: supporting ‘meaningful activity’ in care homes for older people in England

The benefits of meaningful activity in later life are well documented. Studies show that being occupied contributes to both physical and mental health as well as quality of life. Research also suggests that activity may be beneficial to people residing in care homes, including people living with dementia. This paper presents findings from a study which used the Adult Social Care Outcomes Toolkit (ASCOT) to measure quality of life in six care homes located in the south-east of England. The study found, like previous ones, that care home residents’ days were characterised by a lack of activity. Drawing on observations, interviews and focus groups with residents and staff from these homes, this paper attempts to understand why care home residents do not engage in meaningful activities. We reject the idea that these low levels of activity are a natural part of the ageing process or that they can be explained by notions of resident choice. Instead, the findings point to both insufficient funding and working practices within care homes as more substantive explanations. These explanations inform a discussion of how the low levels of engagement in meaningful activity could be addressed and residents’ quality of life improved.

This article reports on a qualitative research study that explored the perspectives and lived experiences of children in a range of New Zealand rural environments. Thirty-six children, aged between 6 and 11 years, were interviewed about living in the country and also contributed artwork and photographs. They came from four specific rural locations, ranging from ‘rural with high urban influence’ to ‘highly rural/remote’. Children expressed positive views about aspects of rural living, such as opportunities for being outdoors and participating in social relationships, confirming a positive discourse of the rural idyll. Their accounts highlighted children's agency under complex and sometimes challenging conditions. Children also, however, experienced some aspects of rural life as dull, dangerous or difficult. The complex and nuanced constructions of rural childhood uncovered in this study point to the critical importance of consulting with children in order to understand their experiences and best meet the needs of rural children and families.

While there is increasing recognition among archaeologists of the extent to which non-agricultural societies have managed their terrestrial ecosystems, the traditional management of marine ecosystems has largely been ignored. In this paper, we bring together Indigenous ecological knowledge, coastal geomorphological observations, and archaeological data to document how Northwest Coast First Nations cultivated clams to maintain and increase productivity. We focus on “clam gardens,” walled intertidal terraces constructed to increase bivalve habitat and productivity. Our survey and excavations of clam gardens in four locations in British Columbia provide insights into the ecological and social context, morphology, construction, and first reported ages of these features. These data demonstrate the extent of traditional maricultural systems among coastal First Nations and, coupled with previously collected information on terrestrial management, challenge us to broaden our definition of “forager” as applied to Northwest Coast peoples. This study also highlights the value of combining diverse kinds of knowledge, including archaeological data, to understand the social and ecological contexts of traditional management systems.

This study investigated the effectiveness of reinforcement procedures in retraining upper extremity movement impaired as a result of cerebrovascular accident. A simply constructed instrument permitted accurate measurement of motor responding and, under some treatment conditions, provided the subject with direct feedback of movement. Experimenter praise for movement which met a pre-selected standard or criterion, in combination with binary visual feedback of performance and a knowledge of progress condition, was shown to bring about continuing improvement in the hemiplegic subject's range of supination. Praise with binary feedback alone functioned only to sustain responding at above baseline levels.

We assessed the impact of a quality improvement intervention to reduce urinary catheter use and associated urinary tract infections (UTIs) at a single hospital. After implementation, UTIs were reduced by 39% (P = .04). Additionally, we observed a slight decrease in catheter use and the number of catheters without an appropriate indication.

Atmospheric and higher pressure RF and microwave plasma sources have numerous applications including material processing and spectroscopy. More recently, advantages in using such discharges for combustion ignition are being investigated. A particularly simple and compact microwave discharge generating device is the quarter wave coaxial cavity resonator (QWCCR). This paper presents a new, compacted design of such a device. A simple approximate analysis of the quality factor, Q, which is a measure of the resonant electromagnetic potential step-up capability is given, and compared to experimentally measured quality factors showing reasonable agreement. Analytic results indicate that the foreshortened folded cavity quality factors are comparable to tapered coaxial cavity designs.

What constitutes a quality research student learning experience? Using an interpretive social sciences approach, the tradition of autoethnography and the interpretive processes of reflexivity and constructivist grounded theory, the individual and collective lived research student experiences of three students were constructed/interpreted and (re)constructed/(re)interpreted to provide insights into answers to this question. Each student had completed one research program and, at the time of publication, was currently enrolled in a doctoral program. Based on these three students' autoethnographies, quality research student experiences engender personal, social, supervisor, research profession, learning communities and other life connectivities. These connectivities are generated through conducive conditions for learning, communities of practice and inclusive research cultures wherein research students experience and do research in order to become researchers, and to belong to research communities.

Summary
Introduction
I met the displaced ‘children of Loxicha’ while carrying out my doctoral fieldwork in Oaxaca City, Mexico, between 2001 and 2002. In Oaxaca City they were simply referred to (by journalists, residents and local non-governmental organisations [NGOs]) as ‘los Loxicha’. Their real home is San Agustín Loxicha, located in the southern region of the state of Oaxaca, as detailed below, but this chapter is based on their time living in a protest camp and a shelter in the city of Oaxaca.
At the time of this research the group was made up of approximately 15 children ranging from age three to fifteen who had been living in the protest camp in the city's central square for four and a half years, where their day-to-day lives included a participatory role in their community's political protest.
Via an exploration of the elements surrounding their day-to-day lives, and the ways in which they are portrayed and viewed by the local media and residents, this chapter seeks to generate debate about how we have come to ‘box’ participation into very clear spaces – spaces that have largely been devised by adults. In particular, the discussion here explores the idea of child participation in relation to its political possibilities, raising such questions as: Who sets the child participation agenda? Which ‘types’ or degrees of child participation are pleasing to adults (and therefore applauded and promoted) and which are disregarded either for their ambiguity or because they represent a threat to the status quo?
The sociopolitical context in Oaxaca from the late 1990s to 2002 provides a volatile backdrop to the lives of the Loxicha children, and is described in the next section.
Oaxaca: political violence and displacement
The primary targets of [the] strategies of militarization are indigenous communities in Chiapas and Oaxaca. The fundamental result is … the self-censorship and fear that has become part of people's lives. And then there are the hard-core human rights abuses including assassinations, kidnappings, torture, rape, and illegal detentions. (Stephen, 2003, p 391)
The region of Oaxaca that is subject to the militarisation described by Stephen above is in the mountainous region between the Sierra Madre del Sur and the Pacific Coast (see Figure 3.1), and includes the Loxicha children's rural home, the community of San Agustín Loxicha.
